CHANGE ORDER <br />PROCEDURE <br />CONTINUED <br />Construction Changes: Only the Owners shall be authorized to make <br />changes to the scope of work for the project. <br />a. All changes shall be documented either as part of an RFI or in the <br />weekly meeting minutes as prepared by the Architect. <br />b. If additional costs and/or time extensions are required, a Proposal <br />Request (on the provided form) shall be prepared by the Architect <br />and submitted to the Contractor for pricing. <br />c. Changes which are given approval in the weekly meeting minutes <br />shall act as the notice to proceed on the work. A note will also be <br />included as to whether or not a Proposal Request will be in order. <br />d. Any instance in which a significant change is made without <br />following the proper procedure shall be reviewed at the monthly <br />"executive" meeting. <br />Proposal Request: Proposal Requests shall be processed by the <br />Architect and Project Manager and submitted to the Contractor when a <br />change in the scope of work required adjustment to the contract amount. <br />a. Upon receipt from the Architect, the Contractor shall distribute the <br />proposal request to the appropriate trade(s) for pricing. <br />Appropriate time extensions, if required, should also be clearly <br />noted in the Proposal Request. <br />b. Once the Contractor has completed the pricing, the Proposal <br />Request will be submitted to the Architect and Project <br />Manager(and engineer if required) for review and approval. Once <br />approved, the Owner will have a chance to review, accept, or reject <br />the Proposal Request. Contractor shall include all necessary <br />°back-up" information for the subcontractors to justify and help in <br />the review by the Architect and/or consultants. <br />c. Once all three parties have reviewed and signed the Proposal <br />Request, the Contractor is authorized to proceed with the work as <br />indicated in the Proposal Request. The Proposal Request will then <br />become a part of a Change order. See attached example. <br />d. Once a Proposal Request is signed by all parties, an original copy <br />will be distributed to each party for their records. <br />e. Contractor shall prepare and distribute an updated Proposal <br />Request log at each of the weekly meetings. <br /> <br />
