SUBMITTALS 3 DEMOLITION PAGE 1 OF 3 1997 1.1 SUBMITTALS AND NOTICES The following items shall be submitted to the Owner or Owner's Representative for review and approval prior to beginning of work: Survey: Before any work is started, the Contractor shall make a thorough survey with the Owner or Owner's Representative in which alterations occur and areas which are anticipated routes of access, and fu 'sh a report, signed by both, to the Contracting Officer. This report shall be listed by building: 1. Existing condition and types of surfaces not required to be altered throughout the areas effected. ~/ 2. Shall designate areas for working space, materials storage and routes of access to areas within buildings where demolition occur. 3. One week prior to th~beginning of the work the Contractor shall provide a written, project~specific work plan signed by Professional Surve in Services" company for the review and approval of the Owner's Representat~ve. s wo p an s all include the number of workers that will be assigned to given work areas, as well as the sequence of operations and anticipated duration or work at specific areas. This plan shall also include a diagram showing the locat~ons of the work ~ areas. 4. A copy of the Contractor's written respirator program that complies with all parts of OSHA Asbestos Regulations CFR Tide 29, Part 1910, Section 1910.1001. ~ 5. A copy of the Contractor's certificate for general liability insurance naming the Owner and his Agents by an "Add~tional Insured Endorsement". ~ 6. A co of the Contractor's license. PY 2 7. Copies of all pemuts necessary to execute and complete the work of this project. ~7 . 8. When necessary, the Contractor shall obtain and submit to the Owne~s Representative the specitic written approval of the Oregon Department of Environmental Quality (and/or other agency havmg jurisdiction) for the equipment or work techniques to be used. 9. Environmental Protection Plan: After the contract is awarded, prior to the commencement of the work, the Contractor shall meet with the Owner or Owner's Representative to discuss the proposed Environmental Protection Plan and to develop mutual understanding relative to details of environmental protection. Not more than 10 days after the meeting, the Contractor shall prepare and submit to the Owner or Owner's Representat~ve for approval, a written and/or graphic Environmental Protection Plan including, but not limited to, the following: a. A list of Federal, State and Local laws, regulations and pemuts concerning environmental protection, pollution control, noise control and abatement that are applicable to the Contractor's proposed operations and the requirements imposed by those laws, regulations imposed by those laws, regulations and pernuts.
