Acknowiedge receipt of this addendum in the space provided on the <br />Form of Proposal. Failune to do so may subject Bidder to disqualification. <br />Soecifications <br />1. Section 00123~ Instrudions to Bidders: <br />a. Add Paragraph 3.1.H as foliows: <br />'Provide camplete infortnadon on qppendices -A', -g•~ 'C' and 'D' to the Form of Proposal, <br />showing lump sum values for each line item. Total up lump sum values in each group for <br />Base Bid and/or for Aftemate No. 3 or Altemate Na 11 and indude in r+espective lump sum, <br />a Fonn of Proposal. Where quantities and un'its are shown i~ the ~efe~+enoed columns, <br />divide the lump sum by the quantity shown to show applicable 'Unit Prices' by line ftem. <br />Quarrtities shown are estimated oniy. Contractor must verify quantities in preparing lump <br />sum elemerrt costs. <br />2. Sedion 00131, Form of Proposal: - <br />a. Delete this sedion in its errtirety and ~eplace with the new Section 00131, FoRn of Proposal, <br />appended to this addendum. <br />3. Sedion 01520, Environmerrtal Practices: <br />a. Add Paragraph 1.6.C as follows: <br />`Emission Rate Standar+ds: As a minimum, all material shall meet the emission rate <br />standarcls set forth below. <br />1.
