An inspection determined that it did not appear adequate to serve the purpose proposed. <br />Based on the results of this study, site approval has been granted for the installation of a <br />new system. This approval is site specific and must be installed according to the following <br />specifications and the attached site plan. <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system, Observe all <br />required setback/separation distances during the installation of the system. <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> <br />TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 150 <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES, MINIMUM: 36INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: ]2INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br />The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br />centers and with no line exceeding 125 feet in length, The trenches and perforated <br />pipe are installed level (+/- one inch) and contoured to follow the natural ground <br />surface, <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952 0010010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility NotiFica <br /> tion Center is (503) 232 1987), <br />2. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON SEPTEMBER <br /> 29, 1999. REFER TO THE EASEMENT: REEL 1635, PAGE 384 WHICH <br /> PROVIDES FOR PLACING THE SEPTIC SYSTEM DRAINFIELDS ON LOT 5, <br /> BLOCK l, MACKEY CREEK ADDITION TO SERVE A NEW RESIDENCE ON <br /> LOT 6, BLOCK 1. MACKEY CREEK ADDITION. SEE ALSO THE SITE <br /> PLAN OF THE PROPOSED DRAINFIELD LOCATIONS DATED SEPTEMBER <br /> 29, 1999. DRAINFIELD CONSTRUCTON SHOULD BE LIMITED TO DRY <br /> SOIL CONDITIONS ON THIS STEEPLY SLOPING LANDFORM. <br />3. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating,the access onto the site. <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected, <br />5, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 373-4427 to request an <br /> inspection, Enter the number "801" for a PRE-COVER inspec- <br /> tion. You may leave a message for the inspector if desired, <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> <br /> <br />
