Based on the results of this study, site approval was granted. The approval is site specific to the area tested, and the septic system must be located in the immediate area of the test pits, unless otherwise authorized by this office~ A new site evaluation may be required if the system is proposed for a different area on the property, This approval was given on the basis that the lot or parcel will not be further partitioned or subdivided and that conditions on this property and on adjacent properties are not altered in any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be altered by cutting~ filling, or other developmental activities which would impede the successful operation of the initial or replacement absorption systems, Failure to comply with the above items may void this approval. The site evaluation approval runs with the land~ but the permit is non-transferable and non refundable. Keep this document with your records, Show this permit to the septic installer prior to installation of the system. Any deviation from the approved plan must have prior approval from this office. The following construction specifications shall apply to the installation of the initial septic system: TYPE OF INITIAL SYSTEM: MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS LINEAL FEET OF DRAINFIELD: 150 TYPE OF DISTRIBUTION: EQUAL DISPOSAL TRENCH DEPTH: 24INCHES. MINIMUM: 24INCHES. MAXIMUM TOTAL DRAIN MEDIA DEPTH: 12INCHES DP~AIN MEDIA UNDER PIPE: 6INCHES MINIMUM SOIL BACKFILL: 12INCHES GROUND WATER INTERCEPTOR: N The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers and with no line exceeding 125 feet in length, The trenches and perforated pipe are installed level (+/- one inch) and contoured to follow the natural ground surface. THE REVIEW IS TO RELOCATE THE SEPTIC TANK AND TO ADD DRAINLINE TO THE EXISTING SYSTEM, THE OLD SEPTIC TANK MUST BE PUMPED AND PROPERLY ABANDONED. ADD AS MUCH DRAINLINE AS POSSIBLE UP TO 150 FEET. SUBMIT A COPY OF THE PUMPING RECEIPT AND THE COMPLETED ABANDONMENT FORM TO BUR OFFICE UPON CONNECTION TO THE NEW TANK. 2. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil
