No <br />evidence of failure was found, and the system appeared to be functioning in a satisfactory manner~ <br /> <br /> This notice is issued with the recommendation that the septic tank be pumped and inspected by <br />a licensed septic tank pumper approximately every five years. <br /> <br /> This notice does not guarantee satisfactory or continuous operation of the existing on-site <br />sewage disposal system, Planning approval may be required for the proposed development of the <br />property. <br /> <br />COMMENIS AND CONUIIiUNS <br /> <br /> 1. CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952~001-0090. You <br /> may obtain co~ies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 222-1987). <br />2. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON JUNE 5, <br /> 2000. THE REVIEW IS IN CONJUNCTION WITH THE NEW RESTROOM <br /> BUILDING. NO APPARENT CONFLICT OR FAILURE WAS NOTICED. AREA <br /> FOR FUTURE REPAIR IS AVAILABLE VIA AN EFFLUENT PUMP TO THE <br /> UPPER PORTION OF THE PROPERTY. THE EXISTING DRAINFIELD IS <br /> PARTIALLY EMCUMBERED BY THE DECK AND DRIVEWAY. THE SEPTIC <br /> TANK SHOULD BE PUMPED ON A REGULAR SCHEDULE, ABOUT EVERY <br /> FOUR TO FIVE YEARS TO EXTEND THE SYSTEM'S USEFUL SERVICE AS <br /> LONG AS POSSIBLE. WE HAVE NO OBJECTION TO THE RESTROOM <br /> BUILDING. IT IS UNDERSTOOD THAT THE SECONDARY STORAGE SHED <br /> WILL NOT BE USED AS AN ADDITIONAL BEDROOM. <br />3. THE ATTACHED OPJNNGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection~ The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected, <br /> <br /> <br />
