No evidence of failure was found and the system appeared to be
functioning in a satisfactory manner. The septic tank was pumped and inspected on 9 /7 /96
by VALLEY SEPTIC

This notice is issued with the recommendation that the septic tank be pumped and inspected by
a licensed septic tank pumper approximately every five years, This notice does not guarantee
satisfactory or continuous operation of the existing on-site sewage disposal system.

Planning approval may also be required for any proposed development of the property.

COMMENTS AND CONDITIONS

1. CALL BEFORE YOU DIG .... IT'S THE LAW.
ATTENTION: Oregon law requires you to follow rules adopted
by the Oregon Utility Notification Center. Those rules are
set forth in OAR 952-001-0010 through OAR 952-001-0090. You
may obtain copies of the rules by calling the center,
(Note: The telephone number for the Oregon Utility Notifica-
tion Center is (503) 232-1987).
2. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON JUNE 17,
1999. THE REVIEW IS IN CONJUNCTION WITH THE PROPOSED
PROPERTY PARTITION. NO APPARENT FAILURE WAS NOTICED. BUT THE
EXISTING DRAINFIELD LINES MIGHT CONFLICT WITH THE NEW DRIVE-
WAY. PUBLIC WORKS MIGHT NOT ALLOW THE DRIVEWAY TO BE CON-
FIGURED TO MISS THE DRAINLINES AS CONFIGURED AS SHOWN BY THE
SITE PLAN DATED JULY 6, 1999. CONSEQUENTLY, THE NEW HOME
MIGHT NEED TO BE MOVED TO MEET MINIMUM SETBACK/SEPARATION
DISTANCES. OUR RECORDS INDICATE THAT THE EXISTING SEPTIC
SYSTEM WAS INSTALLED AUGUST 30, 1980. IT INCLUDES A 1200
GALLON CONCRETE TANK AND 375 FEET OF DRAINFIELD. AN ALTER-
NATIVE SAND FILTER SYSTEM MAY BE NECESSARY FOR FUTURE REPAIR
3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any
vehicular traffic, livestock and farming ~ctivities. Other
activities such as logging, excavating or filling, driveway
placement or any similar activities may also damage the soil
in the area and.adversely affect the septic system approval.
The areas of the initial septic system and the future repair
system must both be protected.
