The <br />inspection revealed conditions which are in conflict with the Department of Environmental Quality <br />On-Site Sewage Disposal Rules, The system conditions are described under the Comments and <br />Conditions listed below. <br /> <br />Since these conditions exist, approval cannot be granted to use the existing on-site sewage <br />system. The use of the system cannot,be approved until the system is brought into compliance with <br />the DEQ Rules. The appropriate steps to bring the system in compliance are also described under <br />the Co~ents and Conditions listed below. <br /> <br />CUMMENIS AND CONUIIIUNS <br /> <br />1. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (505) 222-1987), <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 4/29/99 AS A <br /> RESULT OF A CITIZEN COMPLAINT OF A FAILING SEPTIC SYSTEM <br /> (SEE A~ACHED LE~ER), A CONNECTION OF A NEW HOME TO THIS <br /> FAILING TANK CANNOT BE ALLOWED, THE CONNECTION AUTHORIZATION <br /> APPLICATION CAN BE CONVERTED TO A SEPTIC REPAIR PERMIT. <br />3, THE A1-FACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval, <br /> The areas of the initial septic system and the future repair <br /> system must both be protected, <br /> <br /> <br />
