No evidence of failure was found and the system appeared to be <br />functioning in a satisfactory manner. The septic tank was pumped and inspected on / / <br />by <br /> <br />This notice is issued with the recommendation that the septic tank be pumped and inspected by <br />a licensed septic tank pumper approximately every five years. This notice does not guarantee <br />satisfactory or continuous operation of the existing on-site sewage disposal system. <br /> <br />Planning approval may also be required for any proposed development of the property, <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. CAL[ BEFORE YOU DIG .... IT'S THE LAW, <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952 001-0090. You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 222-1987). <br />2. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />3, THIS SEPTIC REVIEW WAS COMPLETED BY PAM BREMER ON 4/30/1999. <br /> There appears to be no adverse impact on the existing <br /> on-site sewage disposal system as reflected by the submitted <br /> septic system certification form. <br /> <br /> <br />
