Based on the results of this study, site evaluation <br />approval was granted. The approval is specific to the area tested, and the septic system must be <br />located in the immediate area of the test pits, unless otherwise authorized by this office. A <br />new site evaluation may be required if the system is proposed for a different area on the property. <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be <br />altered or modified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems. Failure to comply with <br />the above items may void this approval. <br /> <br />This site evaluation approval runs with the land. but the permit is non-transferable and <br />non-refundable, <br /> <br />Keep this document with your records. Show this permit to the septic installer prior to <br />installation of the system. Any deviation from the approved plan must have prior approval from <br />this office, <br /> <br />The following construction specifications shall apply to the installation of the initial <br />septic system: <br /> <br />TYPE OF INITIAL SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 150 <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES. MINIMUM: 30INCHES. MAXIMUM <br /> TOTAL DI~qIN MEDIA DEPTH: 12INCHES <br /> DIe, IN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br />The disposal trenches are constructed (2) feet wide and on mimimum ten (10) foot centers <br />and with no line exceeding 125 feet in length. The trenches and perforated pipe are <br />installed level (+/- one inch) and contoured to follow the natural ground surface. <br /> <br />TYPE OF FUTURE REPLACEMENT SYSTEM: STANDARD <br /> <br />CDMMENTS AND CONDITIONS <br /> <br />1. THE <br /> SITE IS APPROVED FOR A STANDARD DRAINFIELD SYSTEM IN THE <br /> AREA OF THE TEST PITS PROVIDED AND EVALUATED ON THIS DATE. <br /> THIS SYSTEM WILL ACOMMODATE UP TO 10 EMPLOYEES. IF FUTURE <br /> ADDITION OF WORKERS ARE PLANNED, IT IS RECOMMENDED THAT MORE <br /> DPJ~INFIELD BE INSTALLED AT THIS TIME. <br />3. THE A~ACHED OPJNNGE CARD MUST BE POSTED AT ME COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />4.
