No evidence of failure was found in the drainfield and the field appeared to be functioning in a satisfactory manner, This Minor Repair Permit is for replacement of the existing septic tank, and inspection of the property determined that a new tank may be installed to replace the existing tank. The following specifications shall apply to the installation of the septic tank: MINIMUM SEPTIC TANK CAPACITY: Call for inspection of the new septic tank after the tank has been installed and prior to backfill. This permit is issued with the recommendation that the septic tank be pumped and inspected approximately every five (5) years. This notice does not guarantee satisfactory or continuous operation of the existing on-site sewage system disposal field. Planning approval may be required for the proposed development of the property. 1, CALL BEFORE YOU DIG .... IT'S THE LAW. ATTENTION: Oregon law requires you to follow rules adopted by the Oregon Utility Notification Center. Those rules are set forth in OAR 952-001-0010 through OAR 952-001-0090. You may obtain copies of the rules by calling the center, (Note: The telephone number for the Oregon Utility Notifica- tion Center is (502) 222-1987). 2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON FEBRUARY 17, 1999. THE REVIEW IS TO CONSIDER REPLACING THE THREE BED- ROOM HOUSE WITH A NEW THREE BEDROOM MANUFACTURED HOME. NO APPARENT FAILURE WAS NOTICED DURING THE REVIEW, EXISTING WELLS AND THE 100 FEET SETBACK BETWEEN DPJ~INFIELDS AND THE WELLS RESTRICTS THE ARF~q FOR FUTURE REPAIRS, THE NEW SEPTIC TANK MUST MEET A FIFTY FEET SETBACK FROM ALL WELLS. THE OLD TANK MUST BE PUMPED AND PROPERLY ABANDONED. SUBMIT A COPY OF THE PUMPING RECEIPT AND COMP[E-FED ABANDONMENT FORM TO OUR OFFICE UPON CONNECTION TO THE NEW TANK. 3, THE A~ACHED OPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection, The card must be located near the driveway to the property to assist the inspector in locating the access onto the site, 4, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval, The areas of the initial septic system and the future repair system must both be protected. 5, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL When work is complete and ready for inspection, call our Automated Inspection Request line at 372-4427 to request an inspection. Enter the number "801" for a PRE-COVER inspec- tion. You may leave a message for the inspector if desired, Be sure the System Record form with the As-Built drawing is on the job or in our office when the inspection is requested An inspection should be completed within seven (7) d~s.
