This permit is issued with the recommendation that the septic tank <br /> be <br />pumped and inspected approximately every five (5) years. <br /> <br />This notice does not guarantee satisfactory or continuous operation of the existing <br />on-site sewage system disposal field. Planning approval may be required for the <br />proposed development of the property. <br /> <br />1, CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (502) 222-1987). <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON FEBRUARY <br /> 17, 1999. THE REVIEW IS TO CONSIDER REPLACING THE THREE BED- <br /> ROOM HOUSE WITH A NEW THREE BEDROOM MANUFACTURED HOME. NO <br /> APPARENT FAILURE WAS NOTICED DURING THE REVIEW, EXISTING <br /> WELLS AND THE 100 FEET SETBACK BETWEEN DPJ~INFIELDS AND THE <br /> WELLS RESTRICTS THE ARF~q FOR FUTURE REPAIRS, THE NEW SEPTIC <br /> TANK MUST MEET A FIFTY FEET SETBACK FROM ALL WELLS. THE OLD <br /> TANK MUST BE PUMPED AND PROPERLY ABANDONED. SUBMIT A COPY <br /> OF THE PUMPING RECEIPT AND COMP[E-FED ABANDONMENT FORM TO <br /> OUR OFFICE UPON CONNECTION TO THE NEW TANK. <br />3, THE A~ACHED OPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />4, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval, <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />5, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 372-4427 to request an <br /> inspection. Enter the number "801" for a PRE-COVER inspec- <br /> tion. You may leave a message for the inspector if desired, <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An inspection should be completed within seven (7) d~s. <br /> <br /> <br />
