An inspection determined that it did not appear adequate to serve the purpose proposed. <br />Based on the results of this study, site approval has been granted for the installation of a <br />new system, This approval is site specific and must be installed according to the following <br />specifications and the attached site plan. <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system, Observe all <br />required setback/separation distances during the installation of the system. <br /> <br />Keep this document with your records, Give a copy of this permit to the septic system <br />installer prior to the installation of the system, Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 275 <br /> TYPE DF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES. MINIMUM: 24INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br /> centers and with no line exceeding 125 feet in length, The trenches and perforated <br /> pipe are installed level (+/- one inch) and contoured to follow the natural ground <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br /> 1. CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> A1-FENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (502) 222-1987). <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 7/24/98, THE <br /> SITE HAS BEEN APPROVED FOR A STANDARD DRAINFIELD REPLACEMENT <br /> IN THE AREA OF THE TEST HOLES, AN EFFLUENT PUMP MAY BE NEED- <br /> ED DUE TO THE APPARENT ELEVATION DIFFERENCE. THE EXISTING <br /> SEPTIC TANK MUST BE PUMPED AND ABANDONED AT THE TIME OF THE <br /> SYSTEM INSTALLATION. SUBMIT THE A~ACHED ABANDONMENT FORM <br /> WITH THE "AS BUILT" DRAWING FORM. <br />2. THE A~ACHED OP~ANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />4, THE APPROVED SEPTIC SYSTEM ARBA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />5. THE A~ACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED <br /> AND RETURNED TO OUR OFFICE, The septic system installer <br /> must complete and sign the form and attach a copy of the <br /> receipt from the septic tank pumper for pumping the tank, <br />6. An EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic <br /> system disposal field in the approved area and at the <br /> required depth.
