Based on the results of this study, <br />site approval has been granted. This approval is specific to the area tested, and the system <br />must be installed in the immediate area of the test pits. <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system. <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD~ 200 <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES, MININUM; 30INCHES, M~J<IMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br /> centers and with no line exceeding 125 feet in length, The trenches and perforated <br /> pipe are installed level (+/- one inch) and contoured to follow the natural ground <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON OCTOBER 5, <br /> 1998. SOIL CONDITIONS ARE SUITABLE TO CONSIDER A STANDARD <br /> SYSTEM REPAIR. THE OLD SEPTIC TANK MUST BE PUMPED AND PROP- <br /> ERLY ABANDONED. CONTOUR THE NEW DRAINLINES TO THE NATURAL <br /> LANDFORM. REFER TO THE SITE PLAN DATED OCTOBER 6. 1998. OB- <br /> SERVE ALL USUAL SEll,ACK AND SEPARTION DISTANCES. THE OLD <br /> WELL HAS BEEN PROPERLY ABANDONED AS PER ITEM # W104405 DATED <br /> SEPTEMBER 23. 1998. SUBMIT A COPY OF THE SEPTIC TANK PUMPING <br /> RECEIPT AND COMPLETED ABANDONMENT FORM TO OUR OFFICE UPON <br /> CONNECTION TO THE NEW SEPTIC TANK. <br />2. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />2. THE APPROVED SEPTIC SYSTEM ARF~A MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />4, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 372-4427 to request an <br /> inspection. Enter the number "801" for a PRE-COVER inspec- <br /> tion, You may leave a message for the inspector if desired, <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An inspection should be completed within seven (7) days. <br /> <br /> <br />
