An inspection determined that it did not appear adequate to serve the purpose proposed, Based on the results of this study, site approval has been granted for the installation of a new system. This approval is site specific and must be installed according to the following specifications and the attached site plan, Soils in the approved area shall not be altered or modified by Cutting, filling, or other activities which would impede the successful operation of the septic system. Observe all required setback/separation distances during the installation of the system. Keep this document with your records. Give a copy of this permit to the septic system installer prior to the installation of the system. Any deviation from the approved plan must have prior approval from this office, The following construction specifications shall apply to the installation of the septic system: TYPE OF REPLACEMENT SYSTEM: STANDARD MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS LINEAL FEET OF DRAINFIELD: TYPE OF DISTRIBUTION: SERI~J- DISPOSAL TRENCH DEPTH: INCHES. MAXIMUM The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers and with no line exceeding 125 feet in length. The trenches and perforated pipe are installed level (+/- one inch) and contoured to follc~the natural ground surface. COMMENTS AND CONDITIONS 1. THIS SEPTIC REVIEW WAS COMPLETED BY lEE GRIMES ON MAY 15. 1998. THIS APPLICATION IS TO RELOCATE THE SEPTIC TANK TO ACCOMDDATE A LARGER THREE BEDROM MANUFACTURED HOME. A COMBO SEPTIC/PUMP TANK IS PROBABLY NECESSARY TO REACH THE UPPER- MOST DP~qINLINE FROM THE NEW TANK LOCATION. OBSERVE ALL USUAL SETBACK AND SEPAP~qTION DISTANCES, THE EXISTING TANK MUST BE PUMPED AND PROPERLY ABANDONED, SUBMIT A COPY OF THE PUMPING RECEIPT AND COMPLETED ABANDONMENT FORM TO OUR OFFICE UPON CONNECTION TO THE NEW TANK. THE LAGER HUME WILL ENCUMBER A PORTION OF THE STANDARD REPLACEMENT DRAINFIELD AREA SO AN ALTERNATIVE SAND FILTER SYSTEM IS NECESSARY FOR FUTURE REPAIRS, 2. THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities, Other activities such as lo~ging, excavating ar filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval, The areas of the initial septic system and the future repair system must bath be protected, 4. An EFFLUENT LIFT PUMP MAY BE REQUIRED to install the septic system disposal field in the approved area and at the required depth. Detailed instructions for dosing tank / effluent pump installations are available in our office, The electrical wiring to the pump and alarm must be installed in accordance with the appropriate Electrical Code, The elec- trical work must be done under a Marion County Electrical Permit and must be inspected and approved by a Marion County Electrical Inspector. The w~rk must be left uncovered and accessible for inspection.The electrical inspection includes the connections to the pump and alarm in the dosing tank.the wiring in the trench and the connections in the structure.
