An inspection determined that it did not appear adequate to serve the purpose proposed, <br />Based on the results of this study, site approval has been granted for the installation of a <br />new system. This approval is site specific and must be installed according to the following <br />specifications and the attached site plan, <br /> <br />Soils in the approved area shall not be altered or modified by Cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system. <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office, <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> <br />TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: <br /> TYPE OF DISTRIBUTION: SERI~J- <br /> DISPOSAL TRENCH DEPTH: INCHES. MINIMUM; <br /> TOTAL DP~qIN MEDIA DEPTH: INCHES <br /> DPJNIN MEDIA UNDER PIPE: INCHES <br /> MINIMUM SOIL BACKFILL: INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br />INCHES. MAXIMUM <br /> <br />The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br />centers and with no line exceeding 125 feet in length. The trenches and perforated <br />pipe are installed level (+/- one inch) and contoured to follc~the natural ground <br />surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY lEE GRIMES ON MAY 15. <br /> 1998. THIS APPLICATION IS TO RELOCATE THE SEPTIC TANK TO <br /> ACCOMDDATE A LARGER THREE BEDROM MANUFACTURED HOME. A COMBO <br /> SEPTIC/PUMP TANK IS PROBABLY NECESSARY TO REACH THE UPPER- <br /> MOST DP~qINLINE FROM THE NEW TANK LOCATION. OBSERVE ALL USUAL <br /> SETBACK AND SEPAP~qTION DISTANCES, THE EXISTING TANK MUST BE <br /> PUMPED AND PROPERLY ABANDONED, SUBMIT A COPY OF THE PUMPING <br /> RECEIPT AND COMPLETED ABANDONMENT FORM TO OUR OFFICE UPON <br /> CONNECTION TO THE NEW TANK. THE LAGER HUME WILL ENCUMBER A <br /> PORTION OF THE STANDARD REPLACEMENT DRAINFIELD AREA SO AN <br /> ALTERNATIVE SAND FILTER SYSTEM IS NECESSARY FOR FUTURE <br /> REPAIRS, <br />2. THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as lo~ging, excavating ar filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval, <br /> The areas of the initial septic system and the future repair <br /> system must bath be protected, <br />4. An EFFLUENT LIFT PUMP MAY BE REQUIRED to install the septic <br /> system disposal field in the approved area and at the <br /> required depth. Detailed instructions for dosing tank / <br /> effluent pump installations are available in our office, The <br /> electrical wiring to the pump and alarm must be installed in <br /> accordance with the appropriate Electrical Code, The elec- <br /> trical work must be done under a Marion County Electrical <br /> Permit and must be inspected and approved by a Marion County <br /> Electrical Inspector. The w~rk must be left uncovered and <br /> accessible for inspection.The electrical inspection includes <br /> the connections to the pump and alarm in the dosing tank.the <br /> wiring in the trench and the connections in the structure. <br /> <br /> <br />
