An inspection determined that it did not appear adequate to serve the purpose proposed. <br />Based on the results of this study, site approval has been granted for the installation of a <br />new system. This approval is site specific and must be installed according to the following <br />specifications and the attached site plan, <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system, Observe all <br />required setback/separation distances during the installation of the system, <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prier approval from this office, <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> <br />TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: lgog GALLONS <br /> LINEAL FEET OF DRAINFIELD: <br /> TYPE OF DISTRIBUTION: EQUAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES, MINIMUM: 24INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br />The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br />centers and with no line exceeding 125 feet in length. The trenches and perforated <br />pipe are installed level (+/- one inch) and contoured to follow the natural ground <br />surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 4/24/98, THE <br /> PROPOSED HOUSE ADDITION IS APPROVED PROVIDING THE EXISTING <br /> DRAINFIELD LINES ARE REMOVED WITHIN 10' OF THE NEW ADDITION <br /> AND BACKFILLED WITH CLEAN SOIL. THE HEADER HAS A 5' SET <br /> BACK REQUIREMENT. ADD DPJ~INFIELD TO THE OTHER END OF THE <br /> SYSTEM TO MAKE UP FOR WHAT IS REMOVED <br /> IT IS RECOMMENDED THE SEPTIC TANK BE PUMPED IF IT HAS NOT <br /> BEEN PUMPED IN THE LAST 5 YEARS. <br />2. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUN~ ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br /> <br /> <br />
