The approval is site specific to the area tested, and the septic system
must be located in the immediate area of the test pits, unless otherwise authorized by this
office. A new site evaluation may be required if the system is proposed for a different
area on the property.

This approval is given on the basis that the lot or parcel will not be further partitioned
or subdivided and that conditions on this property and on adjacent properties are not altered in
any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be
altered or modified by cutting, filling, er other developmental activities which would impede the
successful o~ration of the initial or replacement soil absorption systems, Failure to comply with
the above items may void this approval.

This report is not a permit to install the sewage system, A permlt must be obtained before
any system construction occurs. A permit may be issued only after the property has been found to
be compatible with applicable LCDC acknowledged local comprehensive land use plan, A Statement of
Land Use Compatibility or its equivalent is required before a construction permit can be issued.

A detailed site plan, drawn to scale (ex: I' = 60"), must be submitted with the permit
application before the construction-installation permit can be issued. The system must be
installed by the property owner or a registered contractor,

This site evaluation report is valid until an on-site sewage system is installed pursuant to the
construction permit obtained from this office, or until cancelation pursuant to the Environmental
Quality Commission Rules.
