SITE EVALUATION APPROVAL ,DATE/TIME : 04/24/98 09:11 APPROVAL NO TYPE : SE STATUS APPROVED PAGE 98-01994 APPROVED 04/24/1998 2 SITE EVALUATION APPROVAL This property has been evaluated for the feasibility of the installation of a septic system for a RESIDENTIAL building site, Based on the results of this study, site evaluation approval is granted. The approval is site specific to the area tested, and the septic system must be located in the immediate area of the test pits, unless otherwise authorized by this office. A new site evaluation may be required if the system is proposed for a different area on the property. This approval is given on the basis that the lot or parcel will not be further partitioned or subdivided and that conditions on this property and on adjacent properties are not altered in any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be altered or modified by cutting, filling, er other developmental activities which would impede the successful o~ration of the initial or replacement soil absorption systems, Failure to comply with the above items may void this approval. This report is not a permit to install the sewage system, A permlt must be obtained before any system construction occurs. A permit may be issued only after the property has been found to be compatible with applicable LCDC acknowledged local comprehensive land use plan, A Statement of Land Use Compatibility or its equivalent is required before a construction permit can be issued. A detailed site plan, drawn to scale (ex: I' = 60"), must be submitted with the permit application before the construction-installation permit can be issued. The system must be installed by the property owner or a registered contractor, This site evaluation report is valid until an on-site sewage system is installed pursuant to the construction permit obtained from this office, or until cancelation pursuant to the Environmental Quality Commission Rules. Subject to the above, this report runs with the land and will automat- ically benefit subsequent owners, Site conditions will require the following installation specifications: TYPE OF INITIAL SYSTEM: SAND FILTER MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS LINEAL FEET OF DRAINFIELD: 125 TYPE OF DISTRIBUTION: EQUAL DISPOSAL TRENCH DEPTH: 1BINCHES. MINIMUM: 18INCHES, MAXIMUM TOTAL DRAIN MEDIA DEPTH: 12 DRAIN MEDIA UNDER PIPE: 6 MINIMUM SOIL BACKFILL: 6 GROUND WATER INTERCEPTOR: N TYPE OF FUTURE REPLACEMENT SYSTEM: SAND FILTER Before any installation takes place, a permit must first be obtained from Marion County. COMMENTS AND CONDITIONS 1, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON APRIL 22, 1998, SOIL CONDITIONS AT THE TWO NEW TEST PITS APPEAR SUIT- ABLE TO CONSIDER ALTERNATIVE SAND FILTER SYSTEM DRAINFIELDS, DRAINFIELDS MUST BE LOCATED WITHIN THE IMMEDIATE AREA OF THE TWO TEST PITS, REFER TO THE REVISED SITE PLAN DATED APRIL 22, 1998. OBSERVE ALL USUAL SETBAC AND SEPARATION DISTANCES, USE CARE NOT TO ALTER THE NATURAL TOPSOIL WITHIN THE DRAIN- FIELD SITES PRIOR TO INSTALLATION. 2, THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located ' near the driveway to the property to assist the inspector in locating the access onto the site. 3, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval.
