Based on the results of this study~ site <br />approval was granted. The approval is site specific to the area tested~ and the septic <br />~ysteM Must be located in the immediate area of the test pits, unless otherwise authorized <br />by this office. A new site evaluation May be required if the system is proposed roi" a <br />diffe"ent area on the property. <br /> <br />This approval was given on *the basis that the lot or parcel will riot be further partitioned <br />or subdivided a~td that conditions on this propertz amd on adjacent properties are not altered <br />in any Manner which would prohibit the issuance oT a permit. Soils in the approved area <br />shall not be altered by cctt~g, filling, or other developmental activities whict wocld <br />:i.~pede the successfuI operation of the initial or replacement absorption systems. Failure <br />to comply with the above items May void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this dOCument with your records. Show this permit to the septic installer' prior to <br />installation of the system. When work is <br /> coMplete, call our Automated Inspection Request Line, <br /> at 37~-4427, to request al'~ "801" PRE-COVER inspection. <br /> Leave a Message for the inspector to i~dicate the name of <br /> the installer and phone number, the si~e of tank installed <br /> and the length of drainfield. An inspe~ction should be <br /> Completed within seveM (7) days. <br />5.
