Based on the results of this study, site evmluatioo <br />approval is granted. The approval is site specific to the area tested, and the septic system <br />must he ].mca~ed in the immediate ~rea of the test pits, unless otherwise authorized by this <br />office. A new site evaluation may be required if the system is proposed for a differ?mt <br />area on the property. <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided an~ that conditions on this property and on adjacent properties are not altered in <br />any manmer which would prohibit the iss~ance ~f a permit. Soils in the approved area shall not be <br />altered or modified by cutting~ fillimg, or other ~evelopmental activities which would impede the <br />successfu~l operation of the inztial or replacement soil absorption systems. Failure to comply with <br />the above items may void this approval. <br /> <br />*;'his report is mot a permit to install the sewage system. A per*mit must be obtained before <br />arty system construction occurs, A permit May be issued only after the property has been found to <br />be compatible with applicable LCI)C acknowledged local comprehensive land use plan. A Statement of <br />Land Use Compatibility or its equivalent is required before a construction permit can be issued. <br /> <br />A detailed site plan, drawn to scale (ex: 1" = 60"), must be submitted with the permit <br />apolication before the construction-~mstallatioo permit can be issued. The system must be <br />in;.~tal].ed by the*property owner or a registered contractor. <br /> <br />This site evaluation report is valid until an on-site sewage system zs installed pursuant to the <br />construction permit obtaine.d, from this office, or until cancelation pursuant to the Emvironmental <br />Quality Commission Rules.
