Based on the results of this study, <br /> site approval has been granted. This approval is specific to the area tested, and the syste~ <br /> Must be installed in the immediate area of the test pits. <br /> <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system. <br /> <br /> Keep this document with your records. Give a ~opy of this permit to the septic system <br /> installer prior to the installation of the system. Any deviation from the approved plan <br /> Must have prior approval from this office. <br /> <br /> The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1888 GALLONS <br /> LINEAL FEET OF DRAIflFIELD: 458 <br /> TYPE OF DISTRIBUTION: EQUAL <br /> DISPOSAL TRENCH DEPTH: 18INCHES, MININUM; 38INCHES, MAXIMUM <br /> TOTAL DRAIN NEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> fllNIMIJM SOIL BACKFILL: 6INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on Minimum ten (18) foot <br /> centers mud with no line exceeding 125 fee~ in ~ength. The trenches and perforated <br /> pipe are installed level (+/- one"inch) ano contoured to follow the natural ground <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY ROBERT FOSTER on <br /> JULY 8. ~9~. <br />2. The attached ORANGE CARD Must be posted at the county road. <br />3. The site Must be protected from traffic~ livestock, farming <br /> or other damaging activities. <br />4. This syst?M Must be iDspeqted prior to cover. To request <br /> an inspection of coMp~eteo worR, call 588-7?84 (24-hour <br /> line). When requesting the inspection, be sure to give <br /> installer's naMe~ phone nuMber~ size of tank, and total <br /> length of drainfseld. InsPectson will be completed within <br /> seven days of request. <br />5. An effluent lift pump May be necessary to construct t~e <br /> disposal field in the approved area and at the correc~ <br /> de~th. <br />6.
