Based on the results of this study, site
apnroval was granted. The approval is site specific to the area tested, and the septic
system must be ].orated in the immediate area of the test pits, unless otherwise authorized
by this office. A new site ~valuation may be required if-the system is proposed for a
different area on the property. .

This approval was given on the basis that the lot or parcel will not be further partitioned
or subdivided and that conditions on this propertx an~ on adjacent properties are not altered
in any manner which would prohibi~ ~e issuance, of ~ permit. Soils in the approved area
sha]! not be altered by c,tting, ~i~zing, or Other oevelopmental activities which would
i~pede the successful operation of the initial or replacement absorption systems. Failure
to comply with the above items may void this approval.
The s~te evaluation approval runs with the land, but the permit is non-transferable and non-
refundable.

Keep this document with your records.
