THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES OH MAY 21, <br /> 1996. INSTALL THE DRIANFIELD AS CURRENTLY FIELD STAKED FOR <br /> [.OCAT~ON. AN EFFLUENT PUMP MIGHT BE NECESSARY TO INSTALL THE <br /> LINES WITHIN THE APPROVED AREA AT THE CORRECT DEPTH. THERE <br /> IS A MODERATE ROCK HATARD ASSOCIATED WITH DRAINFIELD INSTALL <br /> AT[ON. REMOVE ROCK LARGER THAN SIX INCHES FROM THE BACKFILL <br /> MATERAIAL. USE SIX INCHES OF STRAW TO PROTECT THE DRAINLINE <br /> PIPING IN EXCSSIVELY STONEY SOIL. O~SERVE ALL SETBACK AND <br /> SEPARATION DISTANCES. MUST PUMP AMP PROPERLY ABANDON THE <br /> EXISTING SEPTIC TANK~ SU~MIT A COPY OF THE PUMPING RECEIPT <br /> AND COMPLETED ABANDONMENT FORM TO OUR OFFICE. SUBMIT A COPY <br /> OF THE WELL ABANDONMENT AS WELL. SEE S~TE PLAN OF <br />2. The ~tached ORANGE CAR[) must be posted at the county road. <br />~.
