August 13, 1993

APPL%CANT:
SITE NUMBER:

ANNETTE BISSELL
93-2632

Your site has been found suitable for a STANDA/~D ON-SITE SEWAGE DISPOSAL
SYSTEM. The following construction specifications shall apply to the installa-
tion of your septic system.

The septic fane shall have a minimum liquid capacity of 1000
gallons.

The projected daily s~wage flow for this project is calculated at
450 qallo~s ~er day. Based on soil conditions observed on this
site, the disposal field will be si~ed at 150 lin~l feet ~r. ~50
gallons of pro, coted daily sewage flow. A minimum of 450 lineal
feet of dimpoxal field will be required fe~ your proposed develop-
ment. The disposal trenches are constructed two (2) fee= wide and
on minimum ten (10) foot centers and with no line exceeding 1~5
feet in length. The trenches and perforated pipe are i~stalled
level (~1") and contoured tO f~llew the natural g~ound surface.

The disposal field shall be constructed with serial dist~tion
with a minimum trench depth of 24 inches and a maximum ~rensh
depth of 24 inches. There mu~t be at least ~ inches of backfill
over the top of the gravel, measured from the natural ground
~urface.

A groundwater intercepter is required as part of this system, This
interceptor trench must be constructed a minimum twelve (12) inches wide by 42
inches deep with 36 inches of clean gravel placed over a four (4) inch smooth
wall perforated pipe. The i~terc~ptor must be located at least ten (10) feet
upslop~ f~om the disposal field and must discharge through a watertight, non-
perforated pipe beside or below the disposal field. A separation of 25 feet is
required frOm the septic tank if the tank l~ located upslope of the grOundwa-
ter inte~cepto~.

An effluent lift pump may be necessary to construct the disposal field
in the approved area and at the correct depth.

This approval is site SDesific tO the area tested and the septic system
must be located in the in~lediate area of the test pits unless otherwise autho-
rized by thi~ office. A new site evaluation may be required if the system is
proposed for a different area on the property,

~PECIAL CONDITIONS:
Construct the drainfields within the immediate area of th~ three most
northerly test pits. The lower pits are too close to the building
excavation. A pre-sonstruction field stakeout is recommended to estab-
lish the losations and elevations of the individual drainlines. ~efer to
stakeout p~ocedure and the setback list for details to complete the
stakeout. A ~ist of licensed installers who can a~ist with the stakeout
i~ enclosed for your convenience. Both the original and replacement
fields should be staked fo~ location. Use care ~ot to alter the ~atural
topsoil within the proposed drai~field sites. DraiBfields can only be
installed into the natural undisturbed topsoil. Submit a detailed tO~
scale-drawn plot plan of the stakeout to our office as soon as possible.
Soil conditions are marginal, So construction should be limited to the
dry soil summer season.

UNTIL A VALID PERMIT ~/AS BE~ ISSUED BY THIS ~FFICE.
