JUly 6, 1994
APFLICANT:

LYON a ~$$0CIATES
94-2225,

Your site has been found suitable for a SAND PILTER ALTERNATI~r~ SEWAGE
DISPOSAL SYSTEM. The following construction specifications shall apply to the
installation of ye~ ee~tic system.

1. The septic tank mhall have a minimum liquid capacity of 1000

The projected daily sewage flow for this p~oJeet is calculated at
~0 gallons Der day. Based on soil conditions observed on this
$ite~ the, disposal field will be sized at ~5 linea,k,,,,,,,fget ~er tS0
~allon~ of projected daily sewage flow. A mini~num O~ 1~ %ineal
fee% of disposal field will he required for your proposed d~velop-
ment. The dls~osal trenches are constructed two (2) feet wide and
on minimum ten (10) foot centers and with no line exceeding 125
feet in length. The ttenohas and ~rforated pipe are installed
level (±l") and contoured to follow the natural ground surface.

3. The disposal ~ield shall be con~tructed with serial dist~ibution
with a minimum trench depth of 24 inches and a maximum trench
depth of ~ inohe~. There must be st least 12 inches of backfill
over the top ~f the gravel, measured from the natural ground

The sand filter system shall be designed and operated as per OA~
340-71-290 through 340-71-305. Detailed ~lans for the ~ystem must
be submitted prior to permit issuance, A building p~rmit i~
required if a ~ite built concrete box is used as the sand filter
container. A pamphlet is available in our office which details the
requirements for a sand filter ~yst~m.

This a~proval iO site s~oifiu to the area tested and the septic system
must be located in the immediate area of the test 9its unless otherwise autho-
rized by this office. A new site evaluation may be require4 if the system is
proposed for a different area on the property.

SPECIAL COND~TIONS:
Construct the drainfields only within the immediate area of the center
test pit. The easterly pit is too clo~e to an existing well. Soil condi-
tion~ are marginal. Use care ~ot to alter th~ natural topsoil prior tO
construction. A preconstrue~£on ~ield stakeout ia recomm~ended to
establish the elevations and locations of the individual drainfield
lines of both systems. The sand filter might oeed ~o be elevated to
achieve gravity flow into the shallow drainfield lines. Refer to the
stakeout procedure and the setback list for details to complete the
stakeout. Filter fabric should be used tO Cover the drain rock in lieu
of pa~r or straw in loa~y soils.

~IL A VK~ID PERMIT HA~ ~q iS.~/~ED, BY THIS OFF]~C~.
