No evidence of failure was
found and the system appeared to be functioning in a
satisfactory manner. The septic tank was pumped on
October 17, 1981 by Stayton Septic Service and the tank
was inspected at that time.

Approval of hardship mobile homes is subject to annual
renewal by Marion County Planning. An application for
a re-evaluation of the septic system will be required
two years from the date of this notice.

COMMENTS: The septic System was installed and inspected
in January of 1972.

This notice does not guarantee satisfactory or con-
tinuous operation of the existing on-site sewage
disposal system.

ROBERT~0STER, REGISTERED SANITARIAN

Senator 8uHdin8

220 Hig}~'~tre~t N£ Sa em, Ofegor~ 111~730~-~eT0
