This application
is to replace the previous two bedroom mobile home with a
new three bedroom manufactured home. No evidence of
failure was found, but the previous mobile home is gone
and the system not currently in use. The septic tank was
pumped on 9/9/94 by AAA Septic Service and the tank was
inspected at that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed septic
tank pumper approximately every five years. This notice
does not guarantee satisfactory or continuous operation
of the existing on-site sewage disposal system. Planning
approval may be required for the proposed development of
the property.

COMMENTS:

Our records indicate that the existing system
was installed 8/6/85 and consists of a 1000
gallon concrete tank and 3-100' drainfield
lines. The system is not currently in use.

Our office hours are Monday through Friday,
Please contact our office if you have
concerning this report.

8:00 to 4:30.
any questions

LEE A.
