AUMSVILLE, OR 97385 <br /> Site Number: 94-4574 <br /> <br />An inspection was made at the above address on 12/30/94 <br />to determine if the existing on-site sewage system locat- <br />ed on the property appeared adequate to serve the purpose <br />described in the attached application. This application <br />is to construct an addition to the existing office. No <br />evidence of failure was found and the system appeared to <br />be functioning in a satisfactory manner. The septic tank <br />was pumped on 12/20/94 by Stayton Septic Services and the <br />tank was inspected at that time. <br /> <br />This notice is issued with the recommendation that the <br />septic tank be pumped and inspected by a licensed septic <br />tank pumper approximately every five years. This notice <br />does not guarantee satisfactory or continuous operation <br />of the existing on-site sewage disposal system. Planning <br />approval may be required for the proposed development of <br />the property. <br /> <br />COM~ENTS: <br /> <br />Our records indicate that the septic system <br />was constructed 12/8/77 and consists of a <br />1000 gallon concrete tank and (3-100') drain- <br />field lines.
