SE, AUI(SVILLE, OR 97325
Site Number: 94-1235

An inspection was made at the above address on 3/31/94 to
determine if the existing on-site sewage system located
on the property appeared adequate to serve the purpose
described in the attached application. This application
is to connect a temporary hardship manufactured home to
the existing septic system. No evidence of failure was
found and the system appeared to be functioning in a
satisfactory manner. The septic tank was pumped on 2/3/94
by A A A Septic Service and the tank was inspected at
that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed septic
tank pumper approximately every two years. This notice
does not guarantee satisfactory or continuous operation
of the existing on-site sewage disposal system. Planning
approval may be required for the proposed development of
the property.

COMMENTS:

The existing septic tank is small, only 500
gallons, so with the additional sewage flow
load it should be pumped on a more than nor-
mal interval. We no have history of the sep-
tic system in our files.

Our office hours are Monday through Friday,
Please contact our office if you have
concerning this report,

LEE A.
