COakley: <br /> <br />Before an evaluation of your proposed septic application can be <br />oompleted, the following must be submitted. <br /> <br />( <br /> <br />Two test holes must be dug before a field inspection aah be <br />made (see attached Test ~ole Requirements). <br />Two detailed site plans, drawn to scale and inoluding all <br />data as highlighted on the attached Instructions for Prepa- <br />ration of a Site Plan. <br />A Marion COunty assessor map of the property. This may be <br />obtained at the Marion County Assessor, Map Room, 5th Floor, <br />Franklin Building, 495 State Street, Salem. <br />Existing septic tank must be pumped by a DEQ licensed <br />pumping contractor and verification submitted (see attached <br />Pumper Form). <br />Drainfields must be staked out according to Stakeout Proce- <br />dure guidelines (~ee attached). <br />A copy of the recorded easement. <br />Septic tank must be abandoned and Septic Tank Abandonment <br />submitted (see attached). <br />Well must b~ abandoned and Well Abandonment submitted (see <br />attached). <br /> <br />Your applications were submitted 12-~9r93 and will remain in process, <br />pending the submittal of the above information~ until ~i30-94. At <br />that time all applications will expire and fses forfeited. <br /> <br />Our office hours are Monday through Friday, 8:00 - 4:30.
