(Call the Corporation <br />Commission at 1-378-4166 or access the Corporation <br />Commission through State Systems Access on your PC.) if the <br />violator is a corporation llst the registered agent,s name <br />and address. If the violator is an assumed business name <br />list all parties of interest and their addresses. If the <br />violator is an individual give complete name and address. <br /> <br />~ 2. in general, what are the violations? <br /> <br />X 3. What did you observe? <br /> <br />When did the violation(s) occur? <br /> <br />Where did the violation occur? (Street address or tax lot, <br />section, township and range.
