A new site evaluation may be required if the system is
proposed for a different area on the property.

SPECIAL CONDITIONS:
Use extreme care in siting buildings, roadways, water lines and other
encumbrances to ensure enough area and appropriate setback/separation
distances from the septic system drainfields. Refer to the enclosed
setback list which outlines minimum separation distances between septic
systems and other features. A pre-construction field stakeout of all
proposed development is recommended for review and approval by our
department prior to application for permits.

THIS IS NOT A PERMIT TO INSTALL THE SEPTIC SYSTEM.
