Smith <br />20852 Filbert St. NE <br />Aurora, Oregon <br /> <br />An inspection was made at the above address on January <br />30, 1989 to determine if the existing on-site sewage <br />system located on the property appeared adequate to <br />serve the purpose described in the attached applica- <br />tion. This application is to replace an existing mobile <br />home. NO evidence of failure was found and the system <br />appeared to be functioning in a satisfactory manner. <br />The septic tank was pumped on January 19, 1989 by <br />Carl's Septic Tank Cleaning, Inc. and the tank was <br />inspected at that time. <br /> <br />This notice is issued with the recommendation that the <br />septic tank be pumped and inspected by a licensed <br />septic tank pumper approximately every five years. This <br />notice does not guarantee satisfactory or continuous <br />operation of the existing on-site sewage disposal <br />system. Planning approval may be required for the <br />proposed development of the property. <br /> <br />COMMENTS: <br /> <br />This notice is approved subject to the <br />conditions of the Department of Environmental <br />Quality Variance Approval (Schedule A is <br />attached). <br /> <br />Our office hours are Monday through Friday, 8:00 to <br />4:30.
