Marion CourtttJ <br /> Building Inspection Division <br /> Onsite Septic Review <br /> OREGON <br /> PUBLIC WOR <br /> KS <br /> Permit Number: 09-02008 Date: July 23,2009 <br /> Project Address: 4396 Viewcrest RD S,SL. <br /> Completed By: Jessica L.Joye,REHS, Onsite Wastewater Specialist <br /> SEPTIC AUTHORIZATION/MAJOR ALTERATION <br /> Existing system information: <br /> The Marion County files have no information as to the size or the location of the existing septic system <br /> serving this property. <br /> Flow increase information: <br /> According to the DEQ rules,the proposed 3-bedroom would not increase the daily sewage flow for this <br /> dwelling. <br /> System specifications: <br /> An evaluation of the test holes dug for soil analysis was conducted on April 15,2009. The conditions <br /> observed during this site evaluation indicate that the area around the evaluated test holes is CAPABLE of <br /> supporting a STANDARD SEPTIC SYSTEM. Under these conditions, a STANDARD ONSITE <br /> SYSTEM is required by Department of Environmental Quality(DEQ)rules. Please refer to the signed <br /> approved site plan dated July 23,2009. <br /> EFFLUENT SEWER:The effluent sewer is the pipe running from the septic tank to the disposal field. <br /> The effluent sewer shall extend at least five(5)feet from the tank.There must be a minimum difference <br /> of eight (8) inches between the invert (flow line) of the septic tank outlet and the invert of the <br /> distribution or drop box outlet. In Equal Distribution,there must be six (6)inches of fall between the <br /> septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two (2)inches of <br /> fall between the septic tank outlet and the drop box inlet. - <br /> We would highly recommend that the existing septic tank be pumped and inspected as part of this repair <br /> permit. It is often observed that one of the causes of a septic system failure is a poorly constructed or <br /> deteriorating septic tank.The septic tank must be watertight and have the inlet and outlet baffles in place <br /> to functionproperly.If the outlet baffle is missingor excessive solids have <br /> accumulated in the tank,the <br /> solid material from the septic tank can wash out to the DRAINFIELD and cause a failure in the system. <br /> A copy of the pumping receipt should be returned to our office after the tank has been pumped. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND <br /> RETURNED TO OUR OFFICE.The septic system installer must complete and sign the form and attach <br /> a copy of the receipt from the septic tank pumping service for pumping the tank. <br /> 555 Court St.NE -- P.O.Box 14500--Salem,Oregon 97309-5036—Phone(503)588-5147—Fax(503)588-7948 <br /> Page 1 of 2 <br />
