Completed By: Jessica L.Joye,REHS, Onsite Wastewater Specialist SEPTIC AUTHORIZATION/MAJOR ALTERATION Existing system information: The Marion County files have no information as to the size or the location of the existing septic system serving this property. Flow increase information: According to the DEQ rules,the proposed 3-bedroom would not increase the daily sewage flow for this dwelling. System specifications: An evaluation of the test holes dug for soil analysis was conducted on April 15,2009. The conditions observed during this site evaluation indicate that the area around the evaluated test holes is CAPABLE of supporting a STANDARD SEPTIC SYSTEM. Under these conditions, a STANDARD ONSITE SYSTEM is required by Department of Environmental Quality(DEQ)rules. Please refer to the signed approved site plan dated July 23,2009. EFFLUENT SEWER:The effluent sewer is the pipe running from the septic tank to the disposal field. The effluent sewer shall extend at least five(5)feet from the tank.There must be a minimum difference of eight (8) inches between the invert (flow line) of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must be six (6)inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two (2)inches of fall between the septic tank outlet and the drop box inlet. - We would highly recommend that the existing septic tank be pumped and inspected as part of this repair permit. It is often observed that one of the causes of a septic system failure is a poorly constructed or deteriorating septic tank.The septic tank must be watertight and have the inlet and outlet baffles in place to functionproperly.If the outlet baffle is missingor excessive solids have accumulated in the tank,the solid material from the septic tank can wash out to the DRAINFIELD and cause a failure in the system. A copy of the pumping receipt should be returned to our office after the tank has been pumped.
