1 Note: Inspection/Maintenance frequency,on this system is at start-up and then once every 12 months.On <br /> existing systems, Maintenance starts on contract signing then once every 12 months. <br /> Standard maintenance,testing and reporting shall be performed during normal business hours Monday through <br /> Friday(excluding holidays)on a pre-scheduled basis as Service Provider deems necessary or advisable. <br /> At each service the System shall be inspected and serviced in accordance with the instructions in the Systems <br /> O&M Manual, Additionally,an effluent quality inspection consisting of a visual assessment of color,turbidity, <br /> and scum overflow and an olfactory assessment for odor shall be performed. <br /> The Service Provider will affix a"For Service,Call " label near the control panel's alarm signal. <br /> Performance of the yearly Inspection/Service visits shall include notification of needed repair, replacement or <br /> addition of parts used In the system. <br /> The Service Provider shall notify the owner in writing if any improper system operation that cannot be remedied <br /> at the time of servicing. The written notification shall include an estimated date of correction. <br /> Service Provider shall perform the following services that are listed below: <br /> 1. Perform a visual inspection of treatment system and drain field <br /> 2. Pull, inspect,clean,and reinstall all removable parts. <br /> 3. Inspect pumps for proper operation. (flow,amps,volts) <br /> 4. Check all hydro-splitters and tech valve <br /> 5. Inspect septic tanks,pump vaults and pump tank for sludge and scum quantity. NOTE: Tanks will be <br /> pumped at owner's expense if required. <br /> 6. Inspect risers and lid for damage and insure those lids are secured properly. <br /> 7. Test alarm system and insure that control panels are operating correctly. <br /> 8. Check drain field area for problems, IE.,surfacing, ponding,traffic <br /> 9. Take all readings from the panels and keep record of all readings <br /> 10. Inform owner if anything needs to be fixed, repaired or pumped. <br /> 11. Pull DEQ required samples and have them tested(if required) <br /> The Contractor may, but shall not be required to,provide services hereunder if the Customer: <br /> makes alterations or modifications to the System,or <br /> misuses the System,or <br /> attaches devices to it not supplied by its original supplier,or <br /> performs or attempts to perform any type of Maintenance services on the system or any portion thereof. <br /> During the term hereof,Customer shall provide the Contractor with access to the System. Access includes <br /> electrical controls&disconnects,and sufficient workspace to perform the necessary maintenance services. <br /> 2.0 TERMS OF AGREEMENT <br /> This Agreement shall be for a period of 24 months from the date of system start-up,or on the date <br /> Contract is signed on an existing system unless otherwise terminated or cancelled by either party as provided <br /> herein. <br />
