Permit Number: 555-19-001532-PRMT Date:March 28,2019 <br /> Completed By: CRH <br /> Project Address: 5295 St Louis Road NE,Gervais <br /> 052W20 00300 <br /> SEPTIC SYSTEM MAJOR ALTERATION <br /> Rural Areas Consideration for Standard System <br /> Approved Area & System Type <br /> Test pits and other site features were evaluated on March 20, 2019. The conditions <br /> observed during this site evaluation indicate that the area around the evaluated test <br /> pits is not capable of supporting a standard system due to depth to a seasonal water <br /> table. Under OAR 340-071-0220, the seasonal water table cannot rise closer than 24 <br /> inches to the native ground surface. At your site conditions associated with saturation <br /> were noted at 18, 19 and 24 inches. <br /> Under these conditions, a standard system is allowed under RURAL AREA <br /> CONSIDERATION in the vicinity of test pits #1 and #3. Property is located in a <br /> designated rural zone (EFU) and meets all conditions for OAR 340-071-0410. System <br /> design and permit conditions depart from temporary water table standard 340-071- <br /> 0220(1)(b)(B) and will function without creating a public health hazard or polluting <br /> public waters. Sites approved under Rural Area Consideration are limited to a <br /> projected daily sewage flow not to exceed 600 gallons per day. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum <br /> of five bedrooms. Peak sewage flow into the system is limited to a maximum of 525 <br /> gallons per day, with an average sewage flow of not more than 263 gallons per day. <br /> Premature failure of the treatment system may occur if either of these flow quantities <br /> is exceeded. If, for some reason, you expect your waste water may exceed these <br /> flows, it is advisable to increase the size of the treatment system. <br /> Plans are approved with notations. Please refer to the signed approved site plan dated <br /> April 9, 2019. Any deviation from the approved plans must be authorized by this office <br /> prior to installation or the permit may be voided. It is the responsibility of <br /> applicant/installer to coordinate these requirements with all others in OAR 340-071- <br /> 0220. <br /> Specific Requirements <br /> TANK RISERS: A watertight riser must be installed on each tank to extend to the <br /> ground surface or above. The riser lids must be gasketed and securely fastened. <br /> AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal <br /> field in the approved area and at the required depth. Detailed instructions for dosing <br /> tank/effluent pump installations are available in our office. The electrical wiring to the <br /> pump and alarm must be installed in accordance with the appropriate Electrical Code. <br /> The electrical work must be done under a Marion County Electrical Permit and must be <br /> inspected and approved by a Marion County Electrical Inspector. The work must be <br /> left uncovered and accessible for inspection. The electrical inspection includes the <br /> connections to the pump and alarm in the dosing tank, the wiring in the trench and the <br /> connections in the structure. <br />
