Septic Authorization 555-23-008681-AUTH Page 2 of 2 <br /> latitL ,soy yxpt ? s is/ice v £'�/ iv3 `"�'/i <br /> a <br /> Other Special Requirement: This approval is for a personal hardship allowance under DEQ rule.When hardship is no longer <br /> needed, connection must be terminated .The current system consists of 415 linear ft of drainline <br /> with a 1000 gallon tank, absolutely any additional increases in flow will require additional tankage <br /> as this system will now be serving a total of 8 bedrooms temporarily.This drainfield may not be <br /> driven over per rule.and this will be looked at during future hardship inspections and connection of <br /> the hardship to the system may be denied if this continues to be a concern.The next inspection <br /> will be in 2028 pictures will need to be turned in of drainfield and tank area as well as area set aside <br /> for a repair area when needed.A site inspection may be required,dependent upon what pictures <br /> show. <br /> THE AUTHORIZATION NOTICE FOR CONNECTION TO A TEMPORARY HARDSHIP DWELLING <br /> TO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site <br /> plan dated 12/18/2023. <br /> PERSONAL HARDSHIP RENEWAL IS REQUIRED EVERY 5 YEARS. <br /> THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping <br /> service approximately EVERY FIVE(5)YEARS. The septic tank is a reservoir for the solid <br /> materials in the sewage waste stream. Excessive accumulation of these solid materials in the <br /> septic tank can cause the solids to wash out of the tank and into the drainfield. This can cause <br /> irreversible damage to the drainfield and can eventually lead to a system failure. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, <br /> livestock and farming activities. Other activities such as logging, excavating or filling,driveway <br /> placement or any similar activities may also damage the soil in the area and adversely affect the <br /> septic system approval. The areas of the initial septic system and the future repair system must <br /> both be protected. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The <br /> well placement is especially critical and must be located at least one-hundred (100)feet from the <br /> approved initial and replacement DRAINFIELD areas. A copy of the DEQ setback requirements is <br /> available in our office. <br /> Conditions of Approval: <br /> Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system.Should the system fail,a repair <br /> permit from DEQ is required. <br /> If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review must <br /> be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR 340-071-0140(3),Table <br /> 9C and any additional information DEQ needs to complete the review. <br /> You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of <br /> the site evaluation report, plans and specifications for the proposed system, specify the rule(s)to which a variance is being <br /> requested,demonstrate the variance is warranted,and include the variance fee in OAR 340-071-140 Table 9C. A variance may <br /> only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions <br /> render strict compliance, unreasonable, burdensome or impractical.
