Septic Authorization 555-23-008681-AUTH Page 2 of 2 latitL ,soy yxpt ? s is/ice v £'�/ iv3 `"�'/i a Other Special Requirement: This approval is for a personal hardship allowance under DEQ rule.When hardship is no longer needed, connection must be terminated .The current system consists of 415 linear ft of drainline with a 1000 gallon tank, absolutely any additional increases in flow will require additional tankage as this system will now be serving a total of 8 bedrooms temporarily.This drainfield may not be driven over per rule.and this will be looked at during future hardship inspections and connection of the hardship to the system may be denied if this continues to be a concern.The next inspection will be in 2028 pictures will need to be turned in of drainfield and tank area as well as area set aside for a repair area when needed.A site inspection may be required,dependent upon what pictures show. THE AUTHORIZATION NOTICE FOR CONNECTION TO A TEMPORARY HARDSHIP DWELLING TO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site plan dated 12/18/2023. PERSONAL HARDSHIP RENEWAL IS REQUIRED EVERY 5 YEARS. THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping service approximately EVERY FIVE(5)YEARS. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank can cause the solids to wash out of the tank and into the drainfield. This can cause irreversible damage to the drainfield and can eventually lead to a system failure. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling,driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The well placement is especially critical and must be located at least one-hundred (100)feet from the approved initial and replacement DRAINFIELD areas. A copy of the DEQ setback requirements is available in our office. Conditions of Approval: Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system.Should the system fail,a repair permit from DEQ is required. If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of the site evaluation report, plans and specifications for the proposed system, specify the rule(s)to which a variance is being requested,demonstrate the variance is warranted,and include the variance fee in OAR 340-071-140 Table 9C. A variance may only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions render strict compliance, unreasonable, burdensome or impractical.
