The fee covers the following: • Creating documentation,records preparation,files and computer entry e Site visit to do initial start up of system&reportingto.theftegulatory Agency. • Service visits at appro?ciMate6 month intervals(total of 4) • Field analysis&testing supplies .• Annual'Operation'&.Maintenance Report sent to the customer St Regulatory Agency • Collecting the annual Regulatory Agency fee to be sent with the annual report to said Agency The fee is to be paid by the Homeowner to A&B Septic within 10 days of invoicing If the fee is not paid,the report will not be sent to the Regulatory Agency and you will be out of compliance • Telemetry monitoring(if connected to a phone line or Ethernet connection) • The fee is non-refundable,however if you sell the property the contract and any fees paid can be transferred to the new owner.
