It is recommended that the tank is pumped sooner.
THE AUTHORIZATION NOTICE TO CONTINUE CONNECTION TO A TEMPORARY HARDSHIP
DWELLING TO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site
plan dated June 12, 2018.
PERSONAL HARDSHIP RENEWAL IS REQUIRED EVERY 5 YEARS,A SITE VISIT IS REQUIRED EVERY
10 YEARS MINIMUM.
THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping service
approximately EVERY FIVE (5) YEARS AND MUST BE PUMPED AT LEAST EVERY 1'EN(10) YEARS.
The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these
solid materials in the septic tank may cause the solids to wash out of the tank and into the drainfield. This may
cause irreversible damage to the drainfield and may eventually lead to a system failure.
THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and
farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar
activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the
initial septic system and the future repair system must both be protected
OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The well
placement is especially critical and must be located at least one-hundred(100)feet from the approved initial and
replacement DRAINFIELD areas.A copy of the DEQ setback requirements is available in our office.
555 Court St.NE — P.O.Box 14500—Salem,Oregon 97309-5036—Phone(503)588-5147—Fax(503)588-7948
Page 1 of 1
