arion County Building Inspection Division <br /> Onsite Wastewater Review <br /> v.,. 0_RECwC)N!1 <br /> PUBLIC WORKS <br /> Activity Number: 555-23-004035-AUTH Date: June 22,2023 <br /> Project Address: 15934 Stormy Drive NE, Silverton <br /> 071E06D002100 <br /> Completed By: Aidan Bammes,REHS <br /> ONSITE WASTEWATER TREATMENT SYSTEM <br /> AUTHORIZATION NOTICE <br /> Temporary Personal Hardship <br /> Existing y <br /> system information: <br /> Marion County records indicate the system serving the existing dwelling was installed in 2007 under permit <br /> #05-09273.It consists of 150 lineal feet of drainfield that is installed in an easement on the adjacent property <br /> (4166 Forest Ridge Rd NE),an Advantex treatment system,and a 1000-gallon tank.The onsite septic system <br /> serves a 3-bedroom primary residence and will serve an additional 1-bedroom hardship dwelling inside a <br /> converted shop. Reference the attached stamped site plan. The septic system was serviced on 5/17/2023, <br /> where it was determined that pumping would not be necessary. A site visit was conducted by this office,no <br /> signs of failure were observed. <br /> THE AUTHORIZATION NOTICE FOR CONNECTION TO A TEMPORARY HARDSHIP DWELLING <br /> TO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site plan dated <br /> June 22, 2023. <br /> PERSONAL HARDSHIP RENEWAL IS REQUIRED EVERY 5 YEARS. <br /> THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping service <br /> approximately EVERY FIVE(5)YEARS. The septic tank is a reservoir for the solid materials in the sewage <br /> waste stream. Excessive accumulation of these solid materials in the septic tank can cause the solids to wash <br /> out of the tank and into the drainfield. This can cause irreversible damage to the drainfield and can eventually <br /> lead to a system failure. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic,livestock <br /> and farming activities. Other activities such as logging, excavating or filling, driveway placement or any <br /> similar activities may also damage the soil in the area and adversely affect the septic system approval. The <br /> areas of the initial septic system and the future repair system must both be protected. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The well <br /> placement is especially critical and must be located at least one-hundred(100)feet from the approved initial <br /> and replacement DRAINFIELD areas.
