RE: Activity 555-21-006746 at 2334 Crooked Finger Rd, Scotts Mills <br /> Colm Willis <br /> DIRECTOR Dear Ms. Purtyman, <br /> Brian Nicholas, P.E. <br /> A field visit made to the above project address to evaluate the site for a major <br /> ADMINISTRATION alteration permit. Please see findings below: <br /> BUILDING Approved Area & System Type <br /> INSPECTION Test pits and other site features were evaluated on June 14, 2021. The <br /> conditions observed during the evaluation indicate that the area around the test <br /> EMERGENCY pits is suitable for a standard, serial distribution drainfield in the area of the <br /> MANAGEMENT evaluated test pits. <br /> ENGINEERING For details of this conditional approval, please see attached field sheet and map. <br /> ENVIRONMENTAL This approval is for a system designed to serve a single family dwelling with a <br /> SERVICES maximum of 1 bedroom. Peak sewage flow into the system is limited to a <br /> maximum of 300 gallons per day, with an average sewage flow of not more than <br /> OPERATIONS 150 per gallons per day. Premature failure of the treatment system may occur if <br /> either of these flow quantities is exceeded.
