Septic Authorization 555-22-006716-AUTH Page 2 of 3 bate Issue 8/10I22 Date Exptnng 8110123 Work Description H DSHIP R NEWA 2 BI3RM HARDSHIP, B RM MAIN Other Special Requirement: Existing system information: Pictures were submitted and the system appears to be functioning satisfactorily as no evidence of system failure was observed. Existing structures meet all required setbacks to the septic system. There is adequate area for future system repair when needed. The existing septic system serving this property was installed and approved in 1989 under permit No. 14373, and includes a 1000-gallon septic tank with 375 lineal feet of drainfield line. The system was designed for a sewage flow of 450 gallons per day. The septic tank pumping and inspection report dated 6/2/21 by ace septic, indicates that the 1000-gallon concrete septic tank is in good operating condition. Flow increase information: The application does not indicate there will be any additional increase in daily sewage flow for this system. THIS PERSONAL HARDSHIP RENEWAL TO CONTINUE CONNECTION INTO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site plan dated August 10/2022 PERSONAL HARDSHIP RENEWAL is required every 5 years. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging,excavating or filling, driveway placement or any similar activities may also damage the-soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected. THE SEPTIC TANK MUST BE PUMPED AND INSPECTED by a licensed septic tank pumping service EVERY FIVE(5)YEARS. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. Parking areas must be located where vehicles will not impact any part of the system. A copy of the DEQ setback requirements is available in our office. Conditions of Approval: Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system.Should the system fail,a repair permit from DEQ is required. If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of the site evaluation report, plans and specifications for the proposed system,specify the rule(s)to which a variance is being requested, demonstrate the variance is warranted,and include the variance fee in OAR 340-071-140 Table 9C. A variance may only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions render strict compliance unreasonable, burdensome or impractical.
