SECTION 1.0: SCOPE OF DUST WIPE SAMPLING 1.1 Scope of Work EnviroLead, LLC performed an evaluation (dust wipe sampling and visual assessment) for lead and asbestos in the fire related residue present at 49930 N Santiam Hwy SE, Idanha, Oregon 97350. Karl Gerhauser, Oregon Certified Lead-based Paint Inspector/Risk Assessor (License# 2046, June 30, 2023) and Susan Gerhauser, Oregon Certified Lead-based Paint Inspector/Risk Assessor(License# 2083, Expiration June 30, 2023), conducted the evaluation on May 17, 2021 and June 22,2021. Regulatory limits of 10 ug/ft2 (Interior Floors), 40 ug/ft2 (Porch Floors), 100 ug/ft2 (Window Sills), 100 ug/ft2 (Window Troughs)were used. SECTION 2.0: FINDINGS 2.1 Site Description The property is located at 49930 N Santiam Hwy SE, Idanha,Oregon,97350 and is a single- family dwelling with one unit.Written permission from the inhabitant was not required to access the property because they were present at the time of the evaluation.The house,built in 2003,is approximately 2314 square feet. SECTION 3.0: CONCLUSIONS We recommend cleaning the underside of the pool table. DISCLAIMER This is our report of a visual survey and lead dust and asbestos wipe sample analysis of the readily accessible areas of this building, as requested by the owner, and tested components. The presence or absence of LSP or LBP hazards or Asbestos hazards applies only to the tested or assessed surfaces on the date of the field visit and it should be understood that conditions may change due to maintenance. The results and material conditions noted within this report were accurate at the time of the evaluation and in no way reflect the conditions at the property after the date of the evaluation.
