Septic Authorization 555-21-001453-AUTH Page 2 of 2 <br /> date Issued /.13/2,1 �`t e Expi g fel get gK: `` ,# 5$ �*IC <br /> f ad <br /> 431:1- <br /> 3ri <br /> r " 7* <br /> WorkiDescpt1on �FIRE AUTHW/FLI]REP3BDRM'RES W/NSFD1 BDRM*SAME OTPRQN1T+ �4 °,t {, <br /> Vfr <br /> Conditions of Approval: <br /> THIS SYSTEM IS AUTHORIZED FOR RECONNECTION UNDER OAR 340-071-205(4). <br /> There will be no increase in flow as the 3 bedroom dwelling is being replaced with a new 1 bedroom dwelling. <br /> Under OAR 340-71-0205 (4),this system is not currently failing and meets current setbacks. <br /> Marion County has records of the installation of this system including a Certificate of Satisfactory Completion <br /> dated July 31, 1989.The as built drawing indicated 150 linear feet of drainline and a 1000 gallon septic tank. <br /> The tank was replaced due to fire damage from the Beachie Creek fire of 2020, under permit no 555-21-001453 <br /> and a distribution box was installed. <br /> A site inspection conducted by Marion County on February 24, 2021 found that previous records and the site <br /> plan are accurate. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic including construction <br /> vehicles, livestock activities. Other activities such as logging, excavating or filling, driveway placement or any <br /> similar activities may also damage the soil in the area and adversely affect the septic system approval. The <br /> areas of the initial septic system and the future repair system must both be protected. <br /> The septic tank should be pumped and inspected by a licensed septic tank pumping service approximately every <br /> five (5) years. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive <br /> accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into <br /> the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. <br /> Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system.Should the system fail,a repair <br /> permit from DEQ is required. <br /> If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review <br /> must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR <br /> 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. <br /> You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of <br /> the site evaluation report,plans and specifications for the proposed system,specify the rule(s)to which a variance is being <br /> requested,demonstrate the variance is warranted,and include the variance fee in OAR 340-071-140 Table 9C. A variance may <br /> only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions <br /> render strict compliance unreasonable,burdensome or impractical.
