  1. I hereby apply for participation in the Traffic Safety Diversion Program.
  2. I have no convictions or have taken diversion for any crime or violation within five years immediately preceding the date of this citation.
  3. I agree to plead No Contest to the traffic violation which I am currently charged.
  4. I agree to pay the non-refundable presumptive fine for the offense which I have been charged. I understand payment must be received before or on the scheduled court date.
  5. I agree to pay the traffic school fee.
  6. I do not have a Commercial Driver License.
  7. I have no other pending motor vehicle citations in this or any other court.
  8. There was not an accident associated with the alleged offense.
  9. This violation is not a Class A violation, a violation associated with lack of motor vehicle insurance, or a Mobile Electronic Device (cellphone) violation.
  10. I am a licensed driver.
  11. I understand there may be times the court cannot access my driving record and I will be responsible to provide a certified copy upon request.
  12. I agree to attend and complete the court mandated traffic course and provide proof of completion within 90 days.
  13. I agree to maintain a good driving record for a period of six months upon approval of the diversion program.
  14. I understand that if I do not comply with the conditions of my diversion, the diversion will be terminated, and I will be convicted of the offense. I also understand there will be NO extensions, NO exceptions.
  15. I will keep the court advised of my current mailing address and telephone number during the 180 day diversion period.
